The facility makes provisions for caring for the medically-related social needs of the resident during their initial placement, extended stay and the discharge. The process will be met by the Social Services Department. A written social history for each resident shall be kept in the medical record and shall be utilized in developing the resident's plan of care. It shall contain information concerning the resident's previous occupation, cultural and educational background. The record also contains interests in and relationships with other people, hobbies, problems and needs. Recognition shall be given to the resident's previous life experiences, their present interests and strengths, preparation and attitude toward admission, and adjustment to his or her present illness or disability. The Licensed Social Worker (LSW) will aid families and residents in their adjustment to nursing home acre.

The LSW acts as an information and referral source for residents, families and staff in addition to providing lists of consultants available for the community as requested.
